Orthopaedic Fracture Pathway

An orthopaedic fracture pathway was established in LTC to provide care for residents without transferring to the emergency department. LTC homes on this pathway are linked with a fracture clinic and will have:

• Rapid upload of images for the Radiologist to read
• Referral to a fracture clinic without transferring to the Emergency Department

Sunnybrook Health Sciences Centre, Unity Health Toronto, and Toronto Western Hospital (UHN) are three fracture clinics that have implemented this pathway.
