A virtual care program

LTC+ is a virtual care program developed in partnership with Long Term Care (LTC) homes in Toronto, acute care hospital and community service. The program supports LTC homes in the Ontario Health Toronto region deliver the highest quality of care for their residents. The program enhances existing services in LTC home by providing access to Medical Specialist Consultations and Nurse Navigation services.
