Step 1: Call the LTC+ Phone Line: 1 (855) LTC-PLUS 

Step 2: Determine whether you’d like to connect with your LTC+ Hospital Hub or the LTC+ Central Hub

Step 3: If connecting with your LTC+ Hospital Hub, choose your hospital from the options provided

Step 4: Connect directly with your LTC+ Hospital hub OR select the service you’d like to connect with between Internal Medicine and Nurse Navigation.
