About LTC+

LTC+ was established to support long-term care providers and residents by preventing unnecessary hospital transfers. In response to the COVID-19 pandemic, we have strengthened our support for front-line Primary Care Providers in LTC homes, focusing on their specific needs and available resources.

With LTC+, Primary Care Providers now have a clear pathway to discuss care goals, available services, and determine if a transfer to acute care is necessary.

The Model of Care

The LTC+ program is a hub-and-spoke model that provides direct access to a suite of services including general internal medicine, specialist care and community resources.

Regional hub hospitals partner with long-term care homes with their local community to provide medical supports and other services.

LTC homes also have access to a Virtual Care Nurse Navigator located at Women’s College Hospital (WCH) who can provide rapid needs assessments and navigate system resources. Regional hub coordination is led by WCH.

Quality Improvement

LTC+ is a quality improvement (QI) project led by the Centre for Quality Improvement and Patient Safety (C-QuIPS) in partnership with Women’s College Hospital. We are dedicated to enhancing value and delivering beneficial services to LTC homes through ongoing engagement and assessment. By employing a continuous quality improvement approach, the LTC+ program remains adaptable to the evolving needs of LTC homes.

Data & Analytics

High-quality data and analytics drive every aspect of the LTC+ program, from design and implementation to quality improvement and evaluation. We use near real-time data on consultations with LTC+ services and transfers from LTC to hospitals. This data is analyzed to guide ongoing changes and enhancements to the LTC+ intervention.
